The human body without water begins to deteriorate within a specific physiological progression. Dehydration at the two-to-three percent level begins to impair cognitive function and physical performance. At five to eight percent, serious symptoms including headache, dizziness, and dramatically reduced physical capacity emerge. At ten percent and above, organ function is compromised. The timeline from last water to serious impairment, in conditions of relative rest (which Ralston largely maintained after his initial liberation attempts confirmed their futility), is measured in days rather than hours.

He reportedly drank his own urine as a hydration measure. This technique is sometimes presented as a survival hack, but its actual physiological value is limited and context-dependent. Urine is hypertonic relative to blood in a dehydrated person — its solute concentration is higher than that of plasma — which means drinking it actually adds to the kidneys’ filtration burden rather than meaningfully replacing lost fluid. It may provide modest hydration in the very short term, particularly in early dehydration when urine is not yet highly concentrated. As a medium-term strategy, it is of limited value and may accelerate certain aspects of physiological deterioration. Ralston was presumably aware of this; his documented management of his situation throughout suggests a reasonably sophisticated understanding of wilderness medicine principles.

He also faced the specific physiological consequences of his arm’s entrapment. Blood flow to the hand and forearm below the point of compression was severely restricted or eliminated from the moment of entrapment. Without circulation, the tissue below the compression point begins to die — a process that is initially reversible if circulation is restored, but becomes irreversible as the duration of ischemia extends. By the time of his eventual liberation, the arm had been without adequate blood supply for more than five days. The medical assessment that followed the rescue was unequivocal: the arm would not have been salvageable even if external rescue had arrived in time to free him without self-amputation.
